In the time between your assessment and your clinic visit, you will receive a pre-appointment phone call. During your phone call, a clinic team member will review the items necessary for your appointment.

Upload Your Documents
Don't forget! As soon as your assessment is completed, you can begin uploading the documents needed for your appointment

Clinic Visit
During your clinic visit, the clinic physician and team members will meet with you to review your information and will assist you with your State of Florida Medical Card application. To view clinic & state fees - click here.

• Review Intake paperwork

• The clinic team submits your Florida application

• You pay your $75 state application fee

Email Proof Of Medical Card
From the date your form is submitted (and $75 state fee paid), it can take about 14 days to receive your confirmation email from the State of Florida with proof of your Medical Card.
