New Medical Card Visit - $200
This covers the cost of your visit and assistance submitting your application to the State of Florida. Upon submission of your application, an additional State of Florida $75 registration fee is due.

State of Florida Medical Card Application
The registration fee is $75 and payable online to the State of Florida, when you application is submitted during your appointment. If you need to pay your application fee at a later date, you may do so. The State of Florida will not begin processing your application until your registration fee has been paid.

10% Military Discount
Active duty military and veterans receive a 10% discount on the $200 new medical card visit.
